The audit will assist clinicians in managing their patients living with diabetes aligned with the Diabetes Annual Cycle of Care. Eligible participants can earn up to **35 CPD hours**. Gestational diabetes is not included.\n\n**Learning Outcomes**\nAt the completion of this CPD activity, participants will be able to:\n1. Identify patients with pathology results consistent with prediabetes and increased risk of progression to type 2 diabetes\n2. Implement structured recall and monitoring processes to support timely follow up and ongoing management of patients with prediabetes and diabetes\n3. Apply individualised glycaemic targets to support the ongoing management of patients with type 1 and type 2 diabetes\n4.
