About

In this webinar, an expert multidisciplinary panel will apply Australian T2D guidance to a framework for identifying and managing patients with T2D who can benefit from clinical review or treatment intensification, from newly diagnosed patients requiring early, individualised management through to those with persistent above-target glycaemia despite established treatment.¹⁻² Through four patient cases, the panel will explore individualised HbA1c targets, early risk-factor management, therapeutic inertia, treatment initiation and intensification, the role of SGLT2 inhibitors (SGLT2i) and GLP-1 receptor agonists (GLP-1 RA), cardiorenal risk, monitoring, and recall.¹⁻²

The webinar features a live Q&A which will allow participants to ask their challenging clinical cases and conundrums. Supported by the expert panel, participants will work through clinical scenarios and receive practical actions that can be applied in comprehensive general practice.

Learning Outcomes

  1. Apply individualised glycaemic targets and risk assessment to identify patients with T2D requiring treatment initiation or intensification, recognising therapeutic inertia.
  2. Select glucose-lowering therapy according to patient characteristics - glycaemic requirements, cardiovascular disease, chronic kidney disease, heart failure, weight and hypoglycaemia risk.
  3. Apply current Australian guidelines and PBS requirements when selecting therapy, including SGLT2 inhibitors and GLP-1 receptor agonists.
  4. Implement monitoring, recall and reassessment strategies for patients with T2D who are overdue for review.

Supported Patient Identification Activity (Measuring Outcomes) - COMING SOON

The Measuring Outcomes (MO) activity is a Patient Identification Activity, with clinic support available from a Wellness Clinical Services Nurse Educator. Express your interest here and the team will be in touch to assess your eligibility for in-clinic or virtual support.
