The overdue patient: Recall, review, treat

Learn about the value of explaining prevention to patients, building planned care around a care plan, and using the practice database to identify and recall patients who are overdue for an HbA1c.

This activity comprises a short educational video, followed by a reflective activity. Participants must complete both components to qualify for 30 minutes of Reviewing Performance (RP) Hours.

1. Outline best practice approaches to T2D patient monitoring and recall in primary care.
2. Critically reflect on your current recall processes and barriers to systematic review to identify optimisation opportunities.
3. Apply a systems-based, team-focused approach to recall and review in your practice.
