A pharmacist referral leads Toni to her GP for a full asthma review, and it turns up a pattern she'd stopped noticing: daily reliever use, night symptoms, activity quietly scaled back. Her GP walks through what changes as a result, a shift to combination preventer-reliever therapy, and why a shared action plan between pharmacy and general practice is what continuity of care actually looks like.

This activity comprises a short educational video, followed by a reflective activity. Participants must complete both components to qualify for 30 minutes of Reviewing Performance (RP) Hours.

Learning Outcomes

  1. Outline a structured approach to identifying and assessing poor asthma control in adults.
  2. Reflect on recent asthma consultations to identify missed indicators of poor control and opportunities for treatment optimisation.
  3. Apply guideline-aligned strategies to optimise asthma management, including ICS-containing therapy, inhaler technique review, asthma action plans, and coordinated care with pharmacy.
