Closing Practice Gaps: Improving T2D in General Practice - Measuring Outcomes Activity

The Measuring Outcomes activity is a Patient Identification Activity designed to help GPs apply learning from the Closing Practice Gaps: Improving T2D in General Practice educational activity to their own practice. Clinic support is available from a dedicated Nurse Educator from the Wellness Clinical Services team. Express your interest here and the team will be in touch regarding next steps.

If you would prefer to complete the activity independently, please download the workbook attached in the resources at the bottom of the page once you’ve completed the evaluation form. We recommend completing the EA activity before commencing the Patient Identification Activity.

After completing the workbook, please click the MO button above to finalise the activity and complete the MO submission. Upon submission, your certificate will be available in your Praxhub history.
